Health Care Plus



New York City and Nassau County have a Mandatory Medicaid managed care program, and most Medicaid consumers will be required to enroll in a Medicaid managed care program. Health Plus is one of several health plans available to consumers in New York City and Nassau County.  Health Care Plus is the name of the Medicaid managed care program offered by Health Plus since 1984. After you enroll in Health Plus, except in a few special cases, you will be required to use doctors, clinics, and hospitals that work with the plan.

REMEMBER: Enrollment in a Medicaid health plan is voluntary until you receive an official notice from either New York Medicaid CHOICE or the Nassau County Social Services Department that you must enroll in a health plan. They can also choose to remain fee-for-service Medicaid until your family receives a mandatory notice from New York Medicaid CHOICE or the Nassau County Social Services Department. Newborns will automatically be enrolled in the health plan of the mother.

Persons receiving AFDC (TANF), SSI, Home Relief, or Medicaid only (MA-only) may be eligible for Health Care Plus. Medicaid is available for some immigrants, including immigrant children with green cards who have been in the country before August 22, 1996. All children who are not eligible for Medicaid can receive Child Health Plus.

Comprehensive Services Include:

  • Doctor's visits(as medically necessary) 
  • All hospital coverage 
  • Emergency Services
  • Over-the-counter and prescription drugs at conveniently located participating pharmacies using your Medicaid card
  • Dental Services 
  • Medical and pediatric health care 
  • Laboratory and X-ray testing 
  • Eye exam and eyeglasses 
  • Hearing tests and hearing aids 
  • Prenatal and Maternal care 
  • Mental Health and Substance Abuse Care 
  • Public transportation reimbursement 
  • Family Planning Services

Health Care Plus additional benefits:

  • In most cases, you will have guaranteed eligibility from Health Plus. This means that you are promised the services that Health Plus covers, along with pharmacy and family planning services, six months after you join, even if you no longer qualify for Medicaid. 
  • 24 – hour Medical Hotline
  • The School Health Program provides medical care to members while they are in school. 
  • The WIC Referral Program provides food to pregnant and nursing women and their children. 
  • Special Events for our community, such as Project A+, encourage children enrolled in Health Plus to do well in school, and Project KidCare teaches parents child safety tips and gives a free photo ID. 
  • Community outreach services for member education. We provide Health Education, including a family-oriented Asthma Education Program, Childbirth Preparation, and Smoking Cessation. 
  • Bilingual physicians and support staff who know your language (Spanish, Arabic, Chinese, Russian, and Polish, among others). 
  • No co-payments or paperwork for additional visits.
