Child Health Plus



Since 1992, Health Plus has offered Child Health Plus - New York State's Health Plan for Kids. Child Health Plus/Health Plus provides a child or teenager with free or affordable health coverage depending on their household income. For most families, Child Health Plus/Health Plus may be FREE or have a small monthly cost. The cost is based on family income and the number of people in the family. Child Health Plus/Health Plus members can choose a family doctor at one of our Health Centers or from among our participating doctors in the community.

The child can also receive services through the School Health Practitioners at selected schools in Brooklyn, Bronx, Manhattan, Queens, and Staten Island.

Children are eligible if they: 

  • Are under the age of 19;
  • Are not eligible for Medicaid;
  • Do not have equivalent health insurance; and
  • Live in Brooklyn, Bronx, Manhattan, Queens, or Staten Island. If they live elsewhere, call (800) 522-5006 to learn the names of their local Child Health Plus insurers.

Child Health Plus provides services for:

  • Primary care services, including well-child care, health education, screening and testing for disease and disability, and treatment of routine childhood illnesses and chronic illnesses
  • Immunizations and inoculations against childhood diseases
  • Hospitalization
  • Dental care
  • Vision Care, including eyeglasses
  • Emergency Room care
  • Ambulances
  • Specialty care services provided at the Health Center and in doctors' offices
  • Clinical laboratory tests, diagnostic x-rays, and other diagnostic procedure
  • Ambulatory radiation therapy, chemotherapy, and hemodialysis
  • Physical and occupational therapy services, limited to 30 visits per year
  • Prescription drugs, insulin, and diabetic supplies when prescribed by a Health Plus doctor and filled by a Health Plus pharmacy
  • Out patient care for the diagnosis and treatment of mental health conditions and for treatment of alcoholism or substance abuse, including comprehensive visits, day visits, and clinic visits. These services are limited to 60 visits per year. A maximum of 20 of the 60 visits may be used for family therapy visits.
  • Durable medical equipment (nebulizer, wheelchairs, canes, crutches, walkers and commode.)

Child Health Plus does not cover :

  • Non Emergency Transportation 
  • Medical supplies (except as specified above)

Home health services except in lieu of inpatient hospital admission or a skilled nursing facility.
