The Child Health Plus recertification team assists families with recertifying their children in Child Health Plus. If you have a child/children in Child Health Plus a member of this team will contact you 90 days prior to the annual recertification date. Health Plus provides the application necessary to recertify your children in Child Health Plus and will assist you in completing the application.
If you are unable to come to a Health Plus location you may request an application by mail or a home visit from a member of the team. All applications must be submitted to Health Plus prior to the renewal date to ensure continuous health coverage.
If your child/family had a change in citizenship status or address since last year, please provide proof of the change with your renewal application. If there has been no change, you do not need to provide proof.
If you are a citizen and your income has remained the same you may use your social security number to attest to your income. If your income has changed or you would like an income review to assess your monthly premium, please provide proof of income with your renewal application.
