Medicaid and Family Health Plus Renewal Information
If you are a Medicaid (Health Care Plus) or Family Health Plus member, you must recertify your health insurance each year.  The New York City Human Resources Administration will send you a renewal package in the mail when it is time for you to recertify.  If you haven’t received your renewal form 90-days before your insurance is due to expire, call us at 1-888-809-8009.

Approximately 90 days before your renewal date, a member of the Health Plus team will contact you to remind you that your annual renewal date is approaching. The staff member will offer assistance in completing the New York City Human Resources Administration renewal package. 

Members who come to one of our locations for assistance will have the package hand delivered to the Human Resources Administration by our team. Members who are unable to make it to a Health Plus location may request a home visit by a member of the Health Plus team or deliver the package to HRA directly.

IMPORTANT: HRA must receive the form by the Respond-by date on the renewal form. Otherwise, they end your Medicaid/Family health Plus benefits and your Health Plus coverage.

Tips for Successful Renewal:


• Look for the Respond-by date on your renewal form – and don’t wait until the last minute to renew!
• If you mail the form to HRA yourself, keep a copy of the form and documents. Don’t send original copies of important documents. And don’t forget to sign the form.
• Never ignore a letter from HRA, even if you recently renewed your coverage or already mailed your form! Sometimes HRA needs more information to make a decision or requires a family to renew before a year has passed.
• If you get a Deferral Notice, call us right away! Act quickly to avoid losing your coverage! Be sure to return the Deferral Notice with the requested proofs to HRA.
