To apply, you will need to meet with a facilitated enroller. He or she will help you complete an application. The facilitated enroller will check your proof documents (residence, age, income, social security number, and citizenship status). You will select a health plan and choose a doctor to take care of you. We are here 7 days a week to help. Call  (888) 809-8009 to make an appointment. Or apply in person in your neighborhood.
